Biobank consent and refusal of those aged 18 and over



In accordance with the Biobank Act, the collection of samples in a biobank requires the donor's written consent and linking of health data to the samples requires written approval. Both can be given on the same consent form.

You also have the right to refuse to participate in biobank activities. You can withdraw your consent and object to the processing of samples and data by signing the refusal form.

You can give consent or make refusal as follows: Fill in the electronic biobank consent or refusal form or print and fill in the desired form and return it by post to the biobank.

If your personal identity code has changed, fill in the refusal form for all personal identity codes you have used.

GIVING YOUR CONSENT OR REFUSAL IN HOSPITAL


You might receive the info sheet, consent and refusal forms in the mail if you have been called in for a medical procedure. In that case, bring the completed consent or refusal form with you to the hospital and give it to the nurse who receives you.

Start by downloading the biobank info sheet and consent form. Once you have read and understood the info sheet, print out and fill in Auria Biobank’s consent form. Return the completed and signed consent form to the address: Auria Biobank, P.O. Box 52, FI-20521 Turku.

PRINTABLE BIOBANK REFUSAL AND WITHDRAWAL OF CONSENT


If you do not want your samples to be used for biobank research at Auria Biobank, download the refusal form, fill it in and send it to the address provided in the form.

You can also withdraw your previously given consent at any time, without giving a reason. Make the withdrawal in writing using the biobank’s refusal form, which you can find on this page. After the biobank has received the withdrawal notice, the biobank will no longer collect your samples or allow access to the samples that are already in the biobank for new research.
