With this form you can cancel biobank consent that you have already given regarding collecting your samples
and/or you can object to the processing of your samples and data by the biobank after a sample has been
taken or proactively before giving consent. The right to object applies to all samples and data and cannot be
restricted or limited.
The cancellation of consent, and/or exercising the right to object, are processed by the biobank as a refusal,
after which your samples and data will no longer be collected, used or assigned for new research. The refusal
will take effect once Auria Biobank has received the signed refusal form.
I understand that after the biobank receives my refusal notification, it can no longer give me information that
was possibly obtained from my biobank sample in biobank research.
